Description: Opioid use disorder (OUD) among women who become pregnant is increasing in prevalence in clinical settings. The standard of care is to provide buprenorphine or methadone as a part of a complete treatment approach during pregnancy and beyond the postpartum period. This course reviews the historical and current context of opioid use of women during pregnancy. It compares pharmacological treatment options for OUD and discusses issues with detoxification or medically assisted withdrawal from opioids during pregnancy. Topics related to the treatment of OUD during pregnancy and in the postpartum period for the mother, fetus, and child will also be discussed (e.g., induction, dosing, pain management, neonatal withdrawal and how to reduce its severity). The learner will follow the case study of Ashley during her pregnancy and the birth of her baby, and will engage in reflection and knowledge check questions.

Educational Objectives:

  • Analyze the historical and current context of pregnant women in the opioid crisis
  • Compare and contrast the differing approaches of treating pregnant patients with opioid use disorder (OUD) with medications for opioid use disorder (MOUD) or using medication to initiate withdrawal
  • Demonstrate the risks and benefits of methadone and buprenorphine treatment during pregnancy for the mother and the child
  • Develop a patient-centered plan to provide postpartum care and plan for any neonatal abstinence syndrome (NAS)/neonatal opioid withdrawal syndrome (NOWS) in the infant

Description: Opioids are commonly prescribed for chronic pain, but are also associated with a risk for serious harms, including overdose and opioid use disorder. This course reviews risk factors for opioid misuse, opioid use disorder, and overdose in patients with chronic pain, explains methods for screening and assessing for problematic opioid use, reviews methods for mitigating risks associated with prescribed opioids, and details non-opioid treatment approaches for pain. The course is based on principles in the Center for Disease Control and Prevention’s guideline on opioids for chronic pain and reviews recent evidence on benefits and harms of opioids.

Educational Objectives:

  • Identify risk factors for opioid misuse, opioid use disorder (OUD), and overdose in patients with chronic pain
  • Practice methods for screening and assessment for problematic opioid use and mitigating risks of opioids
  • Implement non-opioid treatment strategies to manage patients with chronic pain
