Initiating Buprenorphine for Patients Using Fentanyl

This webinar will review variations on buprenorphine dosing that can be considered for patients with opioid use disorder involving fentanyl. The presenters will review the evidence for and describe low dose buprenorphine initiation of protocols, high dose buprenorphine initiation of protocols, approaches to help support patients stop using full agonist opioids for a longer period of time, and a clinical approach to referring patients to alternative medications for opioid use disorder.

20: Treating Pregnant People for Opioid Use Disorder: Clinical Challenges

Opioid use disorder (OUD) among women and people who become pregnant is increasing in prevalence in clinical settings. The standard of care is to provide buprenorphine or methadone as a part of a complete treatment approach during pregnancy and beyond the postpartum period. This module reviews the historical and current context of opioid use among women and birthing people during pregnancy. It compares pharmacological treatment options for women and people who become pregnant with an OUD and also discusses the issues with detoxification or medically assisted withdrawal from opioids during pregnancy. Issues related to the treatment of OUD during pregnancy and in the postpartum period for the birthing person, fetus and child will also be discussed (e.g., induction, dosing, pain management, neonatal withdrawal and how to reduce its severity).

The National Center on Substance Abuse and Child Welfare (NCSACW) is highlighting resources on best practices in the treatment of opioid use disorders and Neonatal Abstinence Syndrome. NCSACW is a national resource center providing information, expert consultation, training and technical assistance to child welfare, dependency court, and substance abuse treatment professionals to improve the safety, permanency, well-being, and recovery outcomes for children, parents, and families.

SAMHSA MAT Guide for Pregnant Women with OUD

SAMHSA, in collaboration with the Administration on Children, Youth, and Families, developed this guide promoting collaborative efforts among agencies and providers serving pregnant and postpartum women with opioid dependence and their infants. This publication includes an overview of the extent of opioid use by pregnant women and evidence-based recommendations for treatment approaches from leading professional organizations.
