Preventing Suicide in People with Opioid Use Disorder

This webinar will review the complex relationships between substance misuse and suicide, with a focus on opioid use disorder (OUD) as a category associated with significant elevation of suicide risk and premature mortality. Methodological challenges in detecting drug poisoning suicides will be reviewed along with emerging clinical data that suggest treatment-seeking OUD patients who have survived non-fatal opioid overdose report desire and intention to die prior to overdose along a continuum of severity, suggesting the utility of suicide screening and prevention integrated into routine care of patients with OUD.

Navigating the Intersection of Migraine and OUD Part 1: Assessment and Non-Opioid Pharmacologic Treatment

This webinar is the first of a 3-part series on the intersection of migraine and opioid use disorder (OUD). Patients with headache commonly report overlapping chronic pain disorders, and may also experience comorbid OUD. Following a brief review of the epidemiology of chronic pain, headache, and OUD, the faculty will describe the intersection of these common conditions, with a focus on comorbid migraine and OUD, particularly in the setting of medication-overuse headache (MOH). An understanding of the pathophysiologic mechanisms underlying pain and headache will help to guide patient assessment and treatment. The faculty will present data to support an opioid-sparing approach to headache due to potential comorbid OUD and multiple other reasons, and describe non-opioid pharmacologic treatments for migraine to consider in the management of patients with comorbid OUD, while acknowledging the importance of treatment for OUD.

Navigating the Intersection of Migraine and OUD Part 2: Interventional, Neuromodulation, and Behavioral Approaches

This webinar is the second of a 3-part series on the intersection of migraine and opioid use disorder (OUD). Chronic headache conditions such as migraine can be treated with multiple non-opioid, non-pharmacologic approaches. The faculty will explore the different types of interventional procedures, non-invasive neuromodulation, and psychological/behavioral approaches that are now available to help manage migraine, while reducing reliance on opioids and other analgesics. Appropriate referral and multimodal integration of such strategies can be especially helpful for patients with chronic headache and comorbid OUD.

Webinar Description: Older adults are often overlooked in public health efforts to address the U.S. opioid epidemic. Yet, in 2018, approximately one quarter of older adults 65 years old and older were prescribed an opioid to treat pain, and were nearly 3 times more likely than younger populations between 20 and 24 years old. As we age, we are more likely to experience chronic pain at a greater severity as well as opioid-related adverse effects like confusion, falls, overdose, and death.

HIV and Substance Use – Intertwined Epidemics

The epidemics of HIV disease and substance use are mutually-reinforcing, and both involve significant morbidity. People with HIV and SUDs have worse health-related outcomes than those with HIV alone. The webinar will review the epidemiology of HIV and substance use, and address the structural and social factors that affect individual risk, particularly within marginalized communities. We will also discuss substance-HIV interactions at the physiologic level, such as neuroinflammatory changes that may worsen HIV- and addiction-related related cognitive deficits and influence the development of chronic pain syndromes. We will discuss recommended approaches to screening and treatment of HIV and SUDs, including the provision of integrated care, MAT, and evidence-based behavioral interventions.

This case-based webinar is the last of a 3-part series on the intersection of migraine and opioid use disorder (OUD). A series of challenging cases will feature patients with migraine whose management includes opioids—used to treat a comorbid chronic pain disorder, migraine, or OUD. The faculty will discuss their approach to each case by applying the multimodal strategies for migraine management introduced in the first 2 webinars of the series. They will navigate the use of medication management, interventional procedures, external neuromodulation, and psychological/behavioral therapies to treat migraine while helping to reduce reliance on opioids; discuss opioid tapering strategies; and address the complexities of managing migraine in patients who require ongoing buprenorphine/naloxone treatment for OUD.
